Applications can be
submitted by mail or applicants can contact the Admissions Office and
make an appointment to visit the campus. A visit to the school is a
great opportunity to tour our facilities, ask questions and meet our
faculty and staff. You may wish to submit your application forms at the
time of a visit or afterwards by mail. Following receipt of correctly
completed application forms, all necessary documents and the application
fee, the Admissions Office will contact you regarding an Enrollment

2. Complete Forms Download the entire application package or individual forms below: Form A Enrollment Application Form Attach photographs of the applicant and parents. Form B Fee Payment Agreement Form C Health Form Form C4 is to be completed by a certified medical practitioner (not a relative of the applicant), no earlier than 3 months prior to submission of this application. Form D Questionnaire & Recommendation Form D2 is to be completed by the applicant’s most recent teacher at the previous school and sent directly from the previous school to the Admissions Office. 3. Gather Additional Documents Academic Transcripts or Report Cards Copies of transcripts for at least the previous full two years must be supplied for all applicants who have previously attended a school or international pre-school. Translations Transcripts in English or Japanese are acceptable, however transcripts in other languages must be provided with an official translation into English by a certified translation company. Passport Copy One copy (personal detail page) of all passports held by the applicant. Payment Documentation The application fee of 15,000 Yen should be made by bank deposit and a copy of the deposit slip included with the application forms. For applications submitted from outside Japan, please contact the Student Fees Office regarding payment methods.

３．その他の必要書類成績表 以前に学校、または、インターナショナルプリスクールに通ったことのある生徒は少なくとも過去2年分の成績表のコピーの提出が必要です。 翻訳 英語または日本語の成績表はそのまま提出して下さい。他の言語で書かれている成績表は翻訳会社で英訳されたものをオリジナルと一緒にお送り下さい。 パスポートのコピー 詳細が記載されているページのコピーを1枚(2冊以上お持ちの方はそれぞれのコピーを1枚)お送り下さい。 入金確認 出願料￥15,000を銀行振り込みでお支払い下さい。その控えを入学書類と一緒にお送り下さい。国外の方の支払い方法は会計にお問い合わせ下さい。
